YOUR PRIVACY PROTECTED

     In most cases your counseling information will not be released to anyone unless you sign a release of information form.

Exceptions to your privacy


     There are only limited situations in which your counseling information will be released without your written consent.  If you use insurance to pay for your counseling a diagnosis must be given to the insurance company.  If you make statements indicating you plan to harm yourself or others an intervention must be made.  If your counseling information is requested in accordance to legal or law enforcement it cannot be held private.  The information released in these cases will be limited to information requested.  In most cases these exceptions do not apply and your counseling information is protected and you control who can see your records.

Right To Review Records


     You have the right to review your counseling records and make additions to your record.  Your request to review your records can take up to sixty days to process.
